It's not always easy to know when a loved one with dementia or Alzheimer's is ready to join Sydney Creek. With a friendly visit, we provide the guidance, answers and support you need during this challenging time. We are here to be a resource to you.
We know that everyone's journey through this process is different. Because each new resident has his or her own rich history and individual needs, Sydney Creek creates a personalized Alzheimer’s or dementia care plan for a smooth and comfortable transition.
Transition
We’ll be with you every step of the way during yours and your loved one’s transition. We practice a team approach to caring for those with dementia. However, here are some proactive points for families to consider to help ensure the smooth continuation of the new life for their loved one:
Prepare yourself—It is important to be realistic and acknowledge the transition may be difficult and that it will take time. Feel free to ask us questions and let us know of any concerns.
Keep open lines of communication—Remember that Sydney Creek is now a part of your team. There might be a few bumps in the road early on while we get to know each other, so it is important for us to speak openly and honestly.
Considerate visits—Families are welcome to visit their loved one often. However, it may be necessary to discuss shorter visits if a resident is having difficulty adjusting to the move. The best times to visit are at meal times and during an activity. This allows your loved one to see you interacting with their new friends, and it also helps facilitate conversation. Please feel free to talk with us as each resident's needs are unique.
Do something for you—If you have been the primary caregiver, you may feel lost initially and grieve the loss of all the hours of care, anticipation and coordination of meeting your loved one's needs. Take time to find out what unmet needs you may have. We all know that the Alzheimer’s and dementia disease process is a long and hard journey, usually lasting many years. The single most important thing you can do to help your loved one for the long haul is to nurture yourself along the way.
Ongoing Support
There are many adjustments to make after a move. As residents get used to their new surroundings at Sydney Creek, they receive ongoing support from compassionate staff members.
Everyone in the Sydney Creek family takes pride in enhancing life for our residents. We share their joys and help them through their challenges. As a family member or loved one, you’ll face some changes too. We provide individual and group support for those times when you just need to talk to someone who understands.
